Cat / Heli Survey, mag launch, help, thanks

Howdy everyone,

We're working on a magazine launch and would like to ask anyone out there with a few minutes to help us with a survey. It's plain software, boring, but seems to get the job done---No prizes for completion, it's short, and you get to tell what you think of mechanized skiing and riding.
