After reading the article,I wondered who didn't live up to their professional responsibility? The Contractor? Consulting Engineers? Or was it DOT (Engineers, Inspector's & Project Management) Employees?
On the surface it appears everyone failed.... It makes me wonder why in the 21st Century, with nearly 100-years of building and maintaining automobile roads, why does this inexcusable practice happen?
